Online Medical Record

View your own medical record online mvr

(You must already be registered for Online Services to be able to view your record)

It is now possible for you to view your Medical Records online.

The system is a website that enables you to view your medical record over the internet from a PC or other mobile devices using the Patient Access App. It allows you to easily and quickly view the electronic medical information held about you by your GP Practice.

Below is a leaflet explaining what the system is, please read this along with the FAQs. Before you can begin using the Online Electronic Health Record (EHR) viewing system we require you to register and consent to use the system in the GP practice, you can go into any reception and ask for one.  

*If you are already registered for Online Services, please complete the 'Online Medical Record Printable Application form' below and return to the practice.

*If you are not already registered for Online Services please read about how to register by clicking here.

Please allow up to 4 weeks for us to process the approval for access to your medical record. Once you have been activated you will receive a letter informing you of the activation.
