If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. There is further information about countries and vaccinations required on the links below
It is important to make this initial appointment as early as possible - at least 8 weeks before you travel - as a follwo up appointment may be required to enable completion of any vaccinations. Some vaccines have to be ordered as they are not a stock vaccine. Please complete the Travel Pre-assessment Questionnaire below and hand into reception as soons as possible.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS. A useful public website for travel health advice is http://www.fitfortravel.nhs.uk

Please note that when requesting completion of any non-NHS document/reports may take upto 10 working days to process.
To claim Statutory Sick Pay for illness of 7 days or less, the patient may self-certify using the appropriate form (SC2) which is available from your employer or on the HMRC website. From the 8th day of sickness we are happy to provide statutory sick pay certificates. If your employer insists on a certificate for the first 7 days we will provide a private certificate. There will be a charge of £18 for this to be paid in advance of the certificate being produced.
Sometimes your GP will want to see you again once your sick note runs out to ensure the problem is sorted and you are fit to return to work in which case your GP will specify this on the sick note and discuss it with you. In most cases you will be assumed to be fit to return to work at the end of the sick note. You can return to work earlier than the sick notes states if your employer is informed and in agreement. There is no longer any note from your GP to say that you are fit to return to work. Sometimes employers ask for a note from your GP to say that you are fit for work. As this is no longer needed it would be something you or your employer would be charged for. The cost would be £18 and this should be paid in advance of the letter being produced.
